    The Re-Entry Coordinator will provide and complete release planning for offenders based on determination that mental health services and community resources will be required when the offender is released from corrections.  

     

    JOB RESPONSIBILITIES INCLUDE:   

    • Maintain compliance with all facility and MHM policies, federal and state laws, regulations and guidelines including HIPAA.
    • Assess the offenders post release needs and the services necessary to meet those needs. 
    • Act as a liaison between Correctional healthcare providers and community agencies and healthcare providers.
    • Contact family and identify other community support systems.
    • Participate in MHM QI program and attend safety trainings.
    • Travel to multiple institutions within the region

    Qualifications

    QUALIFICATIONS:

     

    • Masters Degree in Psychology, Social Work, Counseling or related field from an accredited educational institution. 
    • Current Maryland license as an LCSW-C.
    • Must maintain current tuberculosis documentation and active CPR.
    • Prior experience in corrections, criminal justice or forensics preferred.
