    Centurion of Florida has an opportunity for a Regional Hepatitis C Case Manager to perform a variety of duties involved in the prevention and control of infectious diseases, specifically Hepatitis C, to include the collection, definition, interpretation and reporting of data in a systematic manner.
    The Regional Hepatitis C Case Manager assists with care plan development and provision of care for offenders with acute and chronic infectious disease and serve as a consultant for nurses, physicians and other healthcare professionals regarding care, tracking and reporting of offenders identified with infectious disease.
    The position is a remote/work-from-home opportunity, however up to 60% of travel will be required to facilities within the Raiford/Ocala, Florida areas.

    Essential Responsibilities:

    • Infection Control/Case Management duties for an assigned territory w/ specific focus on the ongoing management of Hepatitis C patients.
    • Supports the work of the site level positions and provides guidance as needed.
    • Consolidates information provided by site level positions for monthly reporting.
    • Works closely with the client in the development and implementation of the programs.
    • Reports directly to the Statewide DON.

    Minimum Qualifications:

    • Must hold valid Florida RN or LPN license
    • CIC (Certification in Infection Control) preferred, not required
    • 1 to 3 years experience providing nursing care in a public health environment or acute care setting with focus on infection prevention and control
    • Proficient in Excel, SharePoint, and various other computer programs
    • Experience in correctional environment preferred
    • Experience working with a high degree of autonomy required
    • Ability to travel throughout assigned territory as required
    • Ability to pass a background check
